The threshold is based on identifying how your needs affect your ability to achieve relevant desired outcomes / goals and whether, as a consequence, this has a ‘significant impact’ on your wellbeing.

If you are assessed as being ‘eligible’ you will be involved in looking at how best to meet your needs and developing a ‘Support Plan.’
Even if you’re found not to have any eligible care and support needs, the assessment process may still help you, for example by providing useful information and advice, linking you with other professionals and organisations, or suggesting other types of community support.
